Handle src -> tgt publish of mcp

1. For mcpTools - we have it twice - can we get rid of mcpServerInformation.json since that info is in the apiinformation.json file?
2. The mcp tools info includes a full path to the operations, but the operations should point to operations on the target apim instance, not the source. We should just do the rewrite for resource ID for the tools. However, we should not interfere with overrides. Either update the tool resource id before overrides (so overrides win) or don't rewrite resourceid if an override for the tool exists.
