docs: record grill decisions and the 2026-07-24 next-run queue#134

A maintainer grill interview settled every pending design decision: iss-35 resolves (its graduation gate is shipped and armed); iss-122 gets a pinned input manifest with tiered depth and procedural-only refusal; itd-93's four open questions resolve (launch sub-verb, self-scaffold parity, built-in rehearsal mode, changelog-heading seam) and it promotes standalone/minor with a PRD from the interview; itd-28's Stage-2 gate is signed off as scanner-seam adapter work (native default, gitleaks opt-in, loud engine reporting). Decisions land in DECISIONS.md with rejected alternatives; the new plans/2026-07-24-next-run-queue.md carries the run queue, STOP conditions, and sequencing for the next autonomous implementation run.
