What is the UX for reporting offensive content?

Scenario: while discovering content, user finds something shocking. How do they tell us about it?

Once use taps "Report Inappropriate" the button state changes to "Cancel Report" as per the MakeAPI's currently supported moderation features. Ideally this would prompt the user to explain why they feel that the app is inappropriate (will file feature ticket on the MakeAPI repo).

As a benchmark, Instagram handles this flow fairly well IMO:
